In order to achieve optimal effects, skin contact is important, so if one has problems with one`s back then it should not hang too loosely. If one has lumbar back problems then it is better to use the back brace. An alternative is to sleep in the T-shirt as that provides good skin contact. If it`s the shoulders one has problems with then the T-shirt may be looser fitting as the T-shirt always has direct contact with the shoulders.
